Convert Classroom Curricula to eLearning ModulesDream House for Medically Fragile Children, Inc. seeks $500,000 in funding to convert its Family for Keeps (TM) Curriculums to eLearning modules that will provide Internet-based acess to training for families, care-givers and non-medically professionals throughout the United States.
Family for Keeps(TM) Transition Care ProgramDream House wrap-around hospital to home Transition Care Programs, Services and Resources: 1) Our one-of-a-kind Transition Care Program provides temporary, family-centered home care for medically fragile children while their own family members (biological, adoptive or foster) and homes are prepared to care for them. 2) Our Education and Skills Training Curriculums (Basic, Advanced and Professional Courses) provide interactive learning experiences teaching families, caregivers, social workers and other non-medical professionals how to provide safe and effective care for medically fragile children outside a clinical setting. 3) Our Bridging the Gap Program helps families acquire therapies, equipment, health care supplies and home modifications to provide safe and accessible homes to meet their children's specialized needs. 4) We recruit, prepare and train respite care providers and foster families for the placement of medically fragile children via our Foster Care Program. 5) The Dream House Advocacy Program strives to heighten public awareness for the plight of medically fragile children by linking private and public sector initiatives to meet the specialized needs of this rapidly growing, extremely vulnerable pediatric population.

We ask for unrestricted financial gifts that will allow us to meet the greatest needs at Dream House January through March 2009. Dream House is a pioneer. What we have created together with our supporters is brand new, with no funding or regulatory precedents to follow. Since inception in 2001, Dream House has: 1) raised more than $3.5 million to care for Georgia's medically fragile children and their families; 2) served more than 950 medically fragile children in 42 Georgia counties; 3) developed a training curriculum unique in the U.S.; 4) set the standard for home care of medically fragile children by demonstrating 100 percent successful outcomes via two pilot programs at the Lilburn Transition Care Home.
